Hospice

27aHospice care can be provided in your private home, in a long term care facility, assisted living or board and lodge, or in a residential hospice house.

 Services are provided by a medically directed interdisciplinary team of professionals and volunteers. Bereavement care is also available to the family following the death of their loved one.

Hospice eligibility criteria includes a 6 month or less life expectancy and the individual is no longer seeking curative treatment. Individuals may stay in the program as long as they continue to meet this criteria, which may exceed 6 months. Hospice is appropriate for any end stage illness (i.e cancer, dementia, heart, kidney, lung or liver disease).
